In this simple example the fields entered, will be carried into the document (Template or Shell) to dynamically render the document (including signature). Other fields (that you dont see) which can be utilized to populate this document would include any inherited during the user's log in (Job Title, Role, Address, Phone, etc.). Here you will notice that certain fields (like First Name, Last Name, Company, Email) were pre-populated. This is because the form fields (within this form) are inheriting those from the user's log in. 

When you submit this record, the returning (Thank You) page will offer you a link to access the document. This Document Shell  (or Template) is actually a MS/Word document (making it easier for you to manage), but the output is Adobe PDF. This is because within the Form Processing (tab), we have set this to 'Convert Word to PDF) on completion. Using MS/Word to manage your many document templates or shells is easier and provides you more functionality to format the output (whether it be MS/Word or PDF output)
